This Confidentiality and Non-Disclosure Agreement (Agreement), dated as of <insert date>, is by and between Blue Cross and Blue Shield of North Carolina (“BCBSNC”), a North Carolina corporation and an independent licensee of the Blue Cross and Blue Shield Association, and <insert full legal name of other entity> a <insert state where other entity is registered> <insert type of entity.> (“<insert other entity’s short form name>”).
