This agreement between Choptank Community Health System, Inc., hereafter referred to as CCHS, Mid-Shore Behavioral Health, a behavioral health core service agency, hereafter referred to as the MSBH, is in reference to expansion of substance use disorder and mental health services in Caroline, Dorchester and Talbot counties via collaboration and education among local primary care and behavioral health providers.
