This AGREEMENT (this “Agreement”) made as of the date set forth in (the "Effective Date") between (hereinafter referred to as the "Group") and Anthem Health Plans of Virginia, Inc. (“Anthem”) upon the following terms and conditions and incorporate by reference certain terms and conditions of the Fully Insured Master Contract (the “Contract”) between the parties.
