The person(s) (“Cardholder”, whether one or more) who signed and returned the Application for a VISA/MASTERCARD, (“Card”) has requested CITY NATIONAL BANK OF TAYLOR (“Issuer”) to extend to Cardholder open-end credit. This Card is being furnished to Cardholder by issuer in reliance on part upon the information supplied by Cardholder in the Application.
