THIS AGREEMENT to provide Claims Administration for the Self-Insured Management and Attorney Dental Plan, (hereinafter referred to as “Contract”), is effective January 1, 2020, by and between the County of Orange, a political subdivision of the State of California, (hereinafter referred to as “County”) and California Physicians’ Service, dba Blue Shield of California , with a place of business at 50 Beale Street, San Francisco, CA 94105, (hereinafter referred to as “Contractor”), which are sometimes individually referred to as “Party” or collectively as “Parties”.
