THIS AGREEMENT is made and entered into on March 18, 2021 by and between Lorain County Public Health, an Ohio corporation (herein after referred to as Company), located at 9880 Murray Ridge Road, Elyria, OH 44035, and CuraLinc, LLC, otherwise known as CuraLinc Healthcare, an Illinois corporation (herein after referred to as CuraLinc), located at 314 W.
