Club Sport, acknowledge that the Club Sports Handbook is available to me at www.luc.edu/campusrec/sports/ sportsclubsforms/ and I certify that I will read and understood the Club Sports Handbook . I fully accept my responsibilities as a player outlined in the Club Sports Handbook and agree to be bound by them. Because of the danger of participating in the above sport, I also recognize the importance of following the rules and regulations established by Loyola University of Chicago and the Department of Campus Recreation and agree to obey any such rules, regulations, and instructions, including the Student Code of Conduct. I understand that failure to comply with these responsibilities may subject me, and/or other team members, to disciplinary action.
