Contract Type FiledOctober 2nd, 2013 Jurisdiction(the “Victim Services Provider”) has elected to participate in the Client Track database, administered by the Indiana Housing and Community Development Authority (“IHCDA”) and therefore is entering into this Participation Agreement for Victim Services Providers (the “Agreement”). Client Track is a database and case management system that collects and maintains information on the characteristics and service needs of clients. The Client Track database is comparable to a Homeless Management Information System, except it is a closed system. The system collects and stores client –level data that can be utilized to generate unduplicated and aggregate reports that can be utilized to determine the use and effectiveness of the services being provided by Victim Services Providers.
