This Service Agreement (“Agreement”), effective as of the date shown below (“Effective Date”), is made and entered into by and among California Hospital Quality Institute (“HQI”), a California not-for-profit corporation, and the hospital(s) listed in Exhibit A (whether single or multiple, hereafter, “Hospital”), which is a member of the California Hospital Association or one of its affiliates. (HQI and Hospital are herein individually sometimes referred to as “Party” collectively as “Parties”).
