THIS CONTRACT is made between Partners Health Management, a local political subdivision of the State of North Carolina (herein known as the LME) contracted with the state of North Carolina to operate a BH I/DD TAILORED PLAN, and [Company] (herein known as the “Provider"), operating under the laws of North Carolina. By means of this Contract, LME is establishing a relationship with health care providers who are reimbursed for State Funded Services by the LME for approved activities and/or services. Each of LME and Provider may be referred to herein as a “Party” and collectively as the “Parties.”
