This Agreement (“Agreement”) is made as of , 20___ by and among PACIFIC LIFE INSURANCE COMPANY (“Pacific Life”), PACIFIC SELECT DISTRIBUTORS, INC. (“Distributor”), a broker/dealer registered with the Securities and Exchange Commission (“SEC”) pursuant to the Securities Exchange Act of 1934, as amended (“Exchange Act”), and a member of the National Association of Securities Dealers, Inc. (“NASD”), (“Broker/Dealer”), and each undersigned agency (jointly and severally referred to herein as “Agency”); Broker/Dealer and Agency jointly and severally hereinafter referred to collectively as “Selling Entities”.
