Community Central Bank Corporation, a Michigan corporation (the “Company”), hereby certifies that, for value received, , or registered assigns, is the registered holder (the “Holder”) of the number of warrants (“Warrants”) set forth above, each Warrant entitling the Holder to purchase twenty (20) share of the Company’s common stock, no par value (the “Shares”), at an exercise price of $5.00 per Share, subject to adjustment as provided in Section 11 herein (the “Exercise Price”), on the terms set forth herein.
