This Pharmacy Services Agreement (the “Agreement”) is effective this 1st day of May, 2007 by and among PRISON HEALTH SERVICES, INC., a Delaware corporation, CORRECTIONAL HEALTH SERVICES, LLC, a New Jersey limited liability company, whose sole member is Prison Health Services, Inc. (hereinafter collectively referred to as “PHS”) and MAXOR NATIONAL PHARMACY SERVICES CORPORATION, a Texas corporation (“Maxor”).
