We have issued this Group Flexible Premium Deferred Variable Annuity Contract (the “Contract”) in consideration of the payment of Purchase Payments, and of any application and/or participation enrollment forms. The Contract Owner is set out on the Contract Specifications page. It is effective as of the Contract Effective Date, which is set out on Contract Specifications page. It is subject to all of the terms and conditions that are set out on the following pages.
