("Company), a domestic corporation, with its principal place of business at 335 Crossways Park Drive, Woodbury, New York 11797, promises to pay to the order of Kathleen Belz, 23 Neel Court, Sayville, N.Y. 11782 at the office of the Company, on the Thirtieth day of September 2001 and pay interest on the unpaid principal of the Note at Eleven & One Quarter (11 1/4) percent per annum, based on 365-day year, semi-annually with the first interest payment being due on the 31st day of December 1996.
