THIS FACILITY AND MANAGEMENT SERVICES AGREEMENT (the “Agreement”), dated as of October 18, 2013, is by and between U.S. Cancer Care, Inc., a Delaware corporation (“Manager”), and 21st Century Oncology LLC, a Florida limited liability company (“Medical Group”). Manager and Medical Group are each individually referred to in this Agreement as a “Party” and, collectively, as the “Parties.”
