THIS AGREEMENT is made this 1st day of October, 2004, by JERSEY SHORE STATE BANK a Pennsylvania-chartered commercial bank located in Jersey Shore, Pennsylvania (the “Company”), and James M. Furey (the “Director”) selected to participate in this Amendment and Restatement of the Jersey Shore State Bank Director Deferred Fee Agreement (the “Agreement”), intending to be legally bound hereby.
