THIS AGREEMENT, made as of November 11, 2016, between Northern Lights Fund Trust IV, an open-end management investment company organized as a Delaware business trust (the “Trust”), and Jefferson National Life Insurance Company of New York, a life insurance company organized under the laws of the State of Texas (the “Company”), on its own behalf and on behalf of each segregated asset account of the Company set forth on Schedule A, as the parties hereto may amend it from time to time (the “Accounts”) (individually, a “Party”, and collectively, the “Parties”) .

This Agreement dated as of the 1st day of May, 2017 is made by and among Jefferson National Financial Corp., on behalf of the life insurance companies listed on Exhibit A (collectively, “Jefferson”) and the current and any future Jefferson separate accounts as applicable (“Variable Accounts”), MainStay VP Funds Trust, a registered investment management company and a Delaware statutory trust (the “Trust”), on behalf of its series (each such fund, a “Fund” and collectively, the “Funds”), and New York Life Investment Management LLC (the “Company”), which serves as manager to the Funds listed on Exhibit B.

This Fund Participation Agreement dated as of the 1st day of May, 2017 is made by and among Jefferson National Financial Corp. (“JNFC”), on behalf of its subsidiaries listed on Exhibit A (collectively, “Jefferson National” unless otherwise indicated) and the current and any future Jefferson National separate accounts as applicable (“Variable Accounts”), Nationwide Fund Distributors LLC (“NFD”), and Nationwide Variable Insurance Trust (the “Trust”).

THIS FUND PARTICIPATION AGREEMENT (“Agreement”) is made as of the 17h day of January, 2017, by and among: Calvert Variable Series, Inc. and Calvert Variable Products, Inc. (each a “Fund,” and together, the “Funds”), each a Maryland corporation, on its behalf and on behalf of each Portfolio (as defined below); Eaton Vance Distributors, Inc. (“EVD”); and Jefferson National Life Insurance Company, a life insurance company organized under the laws of the State of Texas, and Jefferson National Life Insurance Company of New York, a life insurance company organized under the laws of the state of New York (together the “Company”) on behalf of itself and certain of its segregated asset accounts (each an “Account” and together the “Accounts”).
