This Incentive Stock Option Agreement, along with the Sucampo Pharmaceuticals, Inc. Duration and Performance-Based Stock Option Incentive Award Summary delivered herewith (the "Award Summary"), once signed by the individual named on the Award Summary (the "Participant"), shall constitute an Agreement made as of the Grant Date (as indicated on the Award Summary), by and between Sucampo Pharmaceuticals, Inc., a Delaware corporation having its principal office at 4520 East-West Highway, Third Floor, Bethesda, MD 20184 (“Sucampo” and with its direct and indirect subsidiaries, the "Company"), and the Participant.
