American Dental Partners, Inc., a Delaware corporation (the “Company”), has granted to (the “Participant”) an option (the “Option”) to purchase shares of the Company’s common stock, $.01 par value (the “Shares”), for a purchase price of $ per Share (the “Option Price”).
