THIS CHANGE IN CONTROL SEVERANCE AGREEMENT (this “Agreement”) is made by and between Assurant, Inc. (the “Company”) and Donald Hamm (the “Executive”), an officer of the Assurant Health Division (as defined in Section 1(d) below), and is effective as of the 1st day of January, 2005.
