This AGREEMENT (the “Agreement”) is made as of (the “Date of Grant”) by and between CHOICEPOINT INC., a Georgia corporation (the “Company”) on behalf of itself and any Subsidiary which is the employer of the Optionee, and (the “Optionee”).
