This Performance Unit Agreement (the or this “Agreement”) is made between Cincinnati Bell Inc. (the “Company” and, together with all of its subsidiary corporations and organizations, the “Employer”) and (the “Employee”) and is effective as of January 26, 2007. By signing this Agreement, the Company and the Employee each agrees to all of the terms of this Agreement.
