THIS CHANGE IN CONTROL AGREEMENT is dated this day of (the “Initial Effective Date”), between Provident Financial Services, Inc. (the “Company”), a Delaware corporation, and the holding company of The Provident Bank (the “Bank”), and (the “Executive”). The Company and the Bank are sometimes collectively referred to as the “Employers”.
