Dean Street, Englewood, NJ 07631 (“JMH”), and JERRICK VENTURES, LLC, a corporation incorporated under the laws of the State of Delaware and located at 202 S. Dean Street, Englewood, NJ 07631 (“JV” and together with JMH, collectively, the “Company”), in favor of ________________________ (the “Secured Party”).
