D/B/A STAYWELL HEALTH PLAN OF FLORIDA, hereinafter referred to as the "Vendor* or the "Health Plan", whose address is 8735 Henderson Road, Renaissance One, Tampa, Florida 33634, a Florida For Profit Corporation, to provide health care services to eligible Medicaid recipients.
