THIS CONTRACT is entered into between the State of Florida, AGENCY FOR HEALTH CARE ADMINISTRATION, hereinafter referred to as the “Agency”, whose address is 2727 Mahan Drive, Tallahassee, Florida 32308, and AMERIGROUP COMMUNITY CARE, INC., hereinafter referred to as the “Vendor” or “Health Plan”, whose address is 4200 W. Cypress Street, Suite 950, Tampa, Florida 33607, a Florida For-Profit Corporation, to provide Health Care Services to Medicaid Beneficiaries.
