THIS PERFORMANCE STOCK UNIT AWARD AGREEMENT (together with the Terms (as defined below) and Exhibit A hereto, this “Agreement”) dated August 19, 2024, by and between MOLINA HEALTHCARE, INC., a Delaware corporation (the “Corporation”), and Joseph M. Zubretsky (the “Participant”), evidences the award of Performance Units (the “Award”) granted by the Corporation to the Participant as to the number of Performance Units first set forth below.

This Amendment of Employment Agreement is entered into as of August 19, 2024, by and between Joseph M. Zubretsky (the “Executive”) and Molina Healthcare, Inc. (the “Employer”).
