THIS PERFORMANCE STOCK UNIT AWARD AGREEMENT (together with the Terms (as defined below) and Exhibit A hereto, this “Agreement”) dated October 16, 2024, by and between MOLINA HEALTHCARE, INC., a Delaware corporation (the “Corporation”), and Mark L.
