This Client Service Agreement (“Agreement”) is entered into (a) by the undersigned Parties, (b) on the date set forth above their signatures, the same to be effective as of the Effective Date as set forth below and as modified pursuant to the terms and conditions set forth herein, and (c) in consideration of the mutual obligations of the Parties. This Agreement includes the Description of Services Provided, State Regulatory Requirements Anticipated to Apply and the Proposal (collectively, the “Addenda”). The following terms and conditions apply to you and us in connection with this Agreement: 1. Definitions.
