This Placement Agent Agreement (“Agreement”) is made by and between eWellness Healthcare Corporation, a Nevada corporation (the “Company”), and ______________ , each a “Placement Agent” and collectively, the “Placement Agents”), as of the date set forth on the signature page hereto.
