This ADVISORY BOARD MEMBER AGREEMENT (the ''Agreement"), is made and entered into as of this 27th day of June 2018 and effective as of July 1, 2018 (the "Effective Date"), by and between G1 Therapeutics, Inc., a Delaware corporation (the "Company"), and you. This Agreement becomes effective after that certain Advisory Board Member Agreement, effective July 1, 2016, by and between the Company and you, which expires by its terms on June 30, 2018.
