This Amended and Restated Change in Control Severance Agreement (the “Agreement”) is made and entered into effective as of July 22, 2019 (the “Effective Date”) by and between Doug Alleavitch (“Executive”) and Nevro Corp. (the “Company”) and amends and restates that certain Amended and Restated Change in Control Severance Agreement entered into between Executive and the Company dated as of May 5, 2016 (the “Prior Agreement”).
