This Agreement is made and entered into as of August 14th, 2015 by and between Alternative Securities Market, LLC (“Trustee” or “Escrow Agent”) and StepOne Personal Health, Inc.. (“Issuer”) for the Securities Offering known as “StepOne Personal Health Preferred Stock”.

Subject to the terms and conditions of the shares of 9% Preferred Convertible Preferred Stock Units (the "Convertible Preferred Stock”) described in the StepOne Personal Health, Inc. Offering Circular dated AUGUST 14th, 2015 (the "Offering"), I hereby subscribe to purchase the number of shares of 9% Convertible Preferred Stock set forth below for a purchase price of $100.00 per share. Enclosed with this subscription agreement is my check (Online “E-Check” or Traditional Papery Check) or money order made payable to "StepOne Personal Health, Inc." evidencing $100.00 for each share of Convertible Preferred Stock Subscribed, subject to a minimum of ONE 9% Preferred Convertible Preferred Stock Unit ($100.00).
