Tatum Blvd., Suite 180, Phoenix, AZ 85028 (hereinafter “NGH”) and Paradise Valley Family Medicine, P.C. an Arizona professional corporation, with an address of 11209 N. Tatum Blvd., Suite 180, Phoenix, AZ, 85028. (hereinafter “Client”) and is effective as of undersigned date (“Effective Date”).
