This Director Stockholder’s Agreement (this “Agreement”) is entered into as ____________, 20__ (the “Effective Date”), between National Vision Holdings, Inc., a Delaware corporation (the “Company”), and the person identified as the signatory on the signature page hereto (the “Stockholder”) (the Company and the Stockholder being hereinafter collectively referred to as the “Parties”).
