This amendment to the Participation Agreement dated as of August 27, 2015, as amended (the “Agreement”), is effective as of the 1st day of January, 2022, among MEMBERS Life Insurance Company (“Company”) on its own behalf and on behalf of segregated asset accounts of the Company that may be established from time to time (individually, an “Account” and collectively, the “Accounts”), DFA Investment Dimensions Group Inc. (“Fund”), Dimensional Fund Advisors LP (“Adviser”), and DFA Securities LLC (“DFAS”) (individually, a “Party” and collectively, the “Parties”). Capitalized terms used herein and not otherwise defined herein shall have the meanings assigned to such terms in the Agreement.

Franklin Templeton Variable Insurance Products Trust (the “Trust”), Franklin/Templeton Distributors, Inc. (the “Underwriter,” and together with the Trust, “we,” “our,” or “us”), MEMBERS Life Insurance Company, and CUNA Brokerage Services, Inc., your distributor (collectively, the “Company” “you” or “your”), on your behalf and on behalf of certain Accounts, (individually a “Party”, collectively, the “Parties”) have previously entered into a Participation Agreement dated May 1, 2016, as amended (the “Agreement”).

(“Contract Distributor”) entered into a certain participation agreement dated November 18, 2015 (“Participation Agreement”), as may be amended. This Amendment (“Amendment”) to the Fund Participation Agreement is entered into as of January [ ], 2022, by and among the Insurance Company, on its own behalf and on behalf of each separate account of the Insurance Company as set forth in the Participation Agreement, as may be amended from time to time (individually and collectively the “Separate Accounts”), the Fund, Lazard and the Contract Distributor (collectively, the “Parties”). Capitalized terms not otherwise defined herein shall have the meaning ascribed to them in the Participation Agreement (defined above).
