Steps for Goal Setting: Ask the parent/guardian if there is a goal they would like to work on in partnership with staff over the next year. Write the goal in the top box. In partnership with the parent, determine what the approximate completion date of the goal will be. In the second box, start breaking down how to reach the goal into obtainable steps. Write down what needs to be done in each step to move closer to completing the goal. Include information about WHO or WHAT is going to help you accomplish the steps.
