The purpose of this program is to implement comprehensive human immunodeficiency virus (HIV) prevention programs to reduce morbidity, mortality, and related health disparities. In accordance with the National HIV/AIDS Strategy (http://www.whitehouse.gov/sites/default/files/uploads/HNAS.pdf), this program will focus on addressing the HIV epidemic in Georgia in four overarching areas: 1) by reducing new infections, 2) increasing access to care, 3) improving health outcomes for people living with HIV, and 4) promoting health equity.

The Health Promotion Initiative aims to promote healthy behaviors throughout Georgia via innovative health promotion strategies that support the establishment of systems, policy and environmental changes that: increase physical activity; increase citizens access to healthy foods; and reduces Georgian’s exposure to second hand smoke.

The purpose of this program is to implement the “Test-Link-Care” Network. This model will identify and promptly link to care persons who are living with HIV but not receiving treatment (including those who are unaware of as well as, those who are aware of their HIV-positive status) and improve patient retention in HIV primary care, through the use of trained Linkage Coordinators and systemic networking among HIV care providers, HIV testing providers and the health department.

PURPOSE: The CAPUS project will explore more efficient and effective systems to improve HIV testing, linkage to and retention in care, and antiretroviral adherence, specifically targeted toward highest risk minority populations. This approach will be informed by interventions to address certain social determinants of health that fuel the HIV epidemic. We will do this by creating a Georgia cascade of care, identifying interventions for each step in the cascade, monitoring changes in the cascade, and evaluating the effectiveness at each step on improving clinical outcomes and health disparities in Georgia.

PURPOSE: The Adolescent Health and Youth Development (AHYD) Program has been awarded funds to collaborate with the Division of Children and Family Services to implement the Personal Responsibility Education Program. PREP provides evidence-based programming to prevent HIV/STIs and teen pregnancy among Georgia’s most vulnerable youth in Cobb, Clayton, Gwinnett, DeKalb, Bibb, Richmond, Dougherty and Chatham counties.

Funds are distributed to the Gwinnett County Board of Health for the benefit of the counties in District 3-4.
