(Insert Co-op Name) retains sole ownership of the iPad and related equipment. The iPad will be issued to staff and directors according to the guidelines set forth in this document. (Insert Co-op Name) retains the right to collect and/or inspect the iPad at any time and to alter, add, or delete installed software or hardware. All iPads must be returned to the (Insert Co-op Name) office at the end of your term or in the event of removal from the Board or resignation all equipment must be returned immediately to the (Insert Co-op Name) office.

Students enrolled in El Segundo Unified School District (ESUSD) will be provided with an Apple iPad for the purposes of facilitating education. The school is leasing the iPads on behalf of its students from Apple; the iPad must be returned upon a student withdrawing, graduating from the school, or at the end of each school year. The school retains all ownership rights of the iPad; the school may inspect the iPad and all stored information at any time with or without notice, and the student should not have an expectation of privacy as to anything stored on, sent by, or received through it.

We (student and parent/guardian) understand that School District 197 students and their parents must sign this agreement before a student can be issued an iPad for take-home privileges. We understand that this agreement is in addition to the District 197 Acceptable Use of the Internet and Electronic Resources policy (524). We will refer to this form if I have further questions or would like to read the details of this policy agreement in its entirety. We understand that other related policies also apply, including the Student Discipline policy (504), the Bullying Prohibition policy (514) and others that can be found on the District 197 website.
