(the “School”) and other valuable consideration, the receipt and sufficiency of which I acknowledge, intending to be legally bound, knowingly and voluntarily executes this liability waiver and indemnification agreement on behalf of the Participant and myself and our respective representatives, heirs, and next of kin, and further represents, covenants, and agrees as follows:

[ ] (“Organization”) to use the District’s facilities, the Organization agrees to release from liability, waive any and all claims, defend and hold harmless the District, its Governing Board, agents, officers, employees and representatives against all actions, suits, claims, or demands, including, but not limited to, liability for any injury, accident, illness, death, loss, damages, or property damage (collectively, “Losses”) regardless of the cause of the Losses, upon or in connection with the Organization’s activities, action or inaction on the District property or use of the District’s property or facilities, regardless of whether such Losses arise from the active or passive negligence of the District, except for any Losses arising from the sole negligence or willful misconduct of the District.
