Vážení rodiče,

pokud bude vyzvedávat Vaše dítě jiná osoba, než osoba uvedená ve vyzvedávacím formuláři, je třeba předat plnou moc či souhlas s vyzvednutím dítěte jinou osobou.

Plná moc či souhlas musí obsahovat:

1. jméno a příjmení osoby, která si smí dítě vyzvednout

2. datum vyzvednutí

3. číslo OP (popř. kopii OP)

4. podpis zákonného zástupce dítěte

Originál plné moci či souhlasu odevzdejte, prosím, v kanceláři školy.

Dear Parents,

When your child will be picked up by another person than the person written in the "Application form", we need  the "Power of attorney"  or the "Agreement" from you so that your child could be picked up by another person.

The Power of attorney or the Agreement must include:

1. name and surname of the person that is allowed to pick up the child

2. date of pick up

3. number of identity card (or copy of identity card)

4. signature of the legal representative of the child

Please submit the original of  the Power of attorney or the Agreement in the school office.
