Admission Packet definition

Admission Packet means, for the purpose of this policy, the documentation that DCBHS requires the case manager to compile and provide to the out-of-home treatment provider prior to or at the time of the child, youth, or young adult’s admission. No out- of-home treatment provider shall require the case manager to provide clinical information dated more than 12 months prior to the date of the child, youth or young adult’s referral. It is the responsibility of the out-of-home treatment provider to obtain any additional documentation required by the provider’s licensing and/or accreditation standards.
