The loan request, which shall specify the use to be made of the loan proceeds, shall be made on the Appropriate Form and submitted to the Loan Administrator, together with such application fee as may be required under procedures adopted by the Loan Administrator.
Such withdrawal election shall be made at least 30 days prior to the effective date of the withdrawal on the Appropriate Form furnished by the Administrative Committee for such purpose.
