CAAF status does not apply to contractor personnel in support of applicable operations within the boundaries and territories of the United States.

  • Authorized prescriber means a physician, dentist, advanced practice registered nurse or physician assistant.

  • Authorized Recipient means (1) the Shareholder of Record, or (2) any other Officially Designated person to whom redemption proceeds may be sent.
