Examples of Basic Dental Services in a sentence

  • Effective the first of the month following six (6) months of continuous service, Regular Full- Time Employees and Regular Part-Time Employees who opted for benefits in accordance with Article 9, Section 5(b), shall be entitled to enrol in a Dental Plan as follows: Plan A: Basic Dental Services paying for eighty percent (80%) of the approved Schedule of Fees.

  • Basic Dental Services (Plan A) paying for 100% of the approved schedule of fees.

  • Basic Dental Services, including diagnostic, preventative, surgical, restorative, prosthetic, endodontic and periodonic services.

  • The forms shall show all Basic Dental Services provided to each Covered Person and the copayment amounts collected from Covered Persons.

  • Effective the first of the month following six (6) months of continuous service, Regular Full-Time Employees and Regular Part-Time Employees who opted for benefits in accordance with Article 9, Section 6(b), shall be entitled to enroll in a Dental Plan as follows: Plan A: Basic Dental Services paying for eighty percent (80%) of the approved Schedule of Fees.

  • A) Basic Dental Services (Plan A) paying for one hundred percent (100%) of the approved schedule of fees.

  • You agree to provide Basic Dental Services to those Covered Persons who have chosen You as their Primary Care Dentist.

  • The Employer agrees to pay seventy-five percent (75%) of the premium cost of a plan based on the following general principles: Plan A - Basic Dental Services - Pays for eighty percent (80%) of approved schedule of fees to a maximum of $3,000 per year, per insured person.

  • If You are a legal representative of a group dental practice or corporation, You represent and warrant for each Dentist in said group dental practice or corporation who provides Basic Dental Services to Covered Persons that he/she meets the above requirements.

  • All regular full-time employees shall participate in a Dental Plan based on the following general principles: Basic Dental Services (Plan A) Plan pays of approved schedule of fees.
