The tumor should be documented on an MRI of the brain (with and without contrast) or by pathological diagnosis. If the covered person is unable to undergo an MRI of the brain (the study is deemed inappropriate for safety reasons such as the presence of metallic foreign bodies; mechanical reasons such as body habitus; or unavailability), then the tumor should be documented by a CT scan of the head, with and without contrast.‌
