Breast reconstruction definition

Breast reconstruction means both of the following:
Breast reconstruction means reconstruction of a breast incident to mastectomy to restore or achieve breast symmetry. The term includes surgical reconstruction of a breast on which mastectomy has been performed and surgical reconstruction of a breast on which mastectomy has not been performed.
Breast reconstruction. Breast reconstruction surgery for the purposes of breast reconstruction post- mastectomy is a covered service for AHCCCS eligible members consistent with AMPM Policy 310-C. Certified Community Health Worker/Community Health Representative Services: A certified Community Health Worker/Community Health Representative (CHW/CHR), who obtains certification through the Arizona Department of Health Services (ADHS) as specified in AAC R9-16-802, may provide AHCCCS-covered member education and preventive services to eligible members. Refer to AMPM Policy 310-W.
