Please send in one of the following:Newly issued driver’s license or a voter ID card with new address. You must send in a 60 day written notice to cancel your membership agreement. A cancellation fee equal to 20% of the total contract balance is required. Payments will continue on the membership agreement until the sufficient proof of move is received. Cancellation due to Medical Condition: Cancellation is allowed for a permanent disability or severe medical condition.
