Client Form definition

Client Form. – means the client form that is used to collect client information for statistical and other purposes with due observance to prevailing legislation relating to the protection of customer privacy
Client Form means the Client form in the format issued by PHL from time to time.
Client Form means the Web pages on the World Wide Web that CharityWeb will develop, host, and maintain for Client according to the specifications set forth in Exhibit C (“Specifications”).

Examples of Client Form in a sentence

  • The customer must understand their rights and obligations as well as the rights and obligations of the brokers specified under the PSX Regulations and the Standardized Account Opening Form, Know Your Client Form, Standardized Sub-Account Opening Form of CDC, and Agreement(s) of Leveraged Products (Margin Trading System, Margin Financing and Securities Lending and Borrowing), where applicable, and any other applicable Rules, Regulations, Guidelines, Circulars etc.

  • EXPENSES Unless otherwise agreed in writing: (a) the Client will bear all expenses set out in the Client Form; and (b) any third party costs incurred by the Service Provider in the course of performing the Services may be billed to the Client, unless specifically agreed otherwise by the parties in writing.

  • GST (a) Unless otherwise indicated, amounts stated in a Client Form or an invoice provided by the Service Provider do not include GST.

  • PAYMENT METHOD The Client must pay Fees using the payment method specified in the Client Form.

  • Unless otherwise indicated, amounts stated in a Client Form do not include GST.

  • No matter which type of research has been undertaken, this section should include details of the most important results, in the light of the research objectives and the backgrounds of the thesis question or research questions.

  • Client shall complete and provide all the information required in the New Client Form attached hereto, which upon completion shall become a part hereof.

  • For any enquiries or complaints, please contact the Service Provider directly, using the contact details specified in the Client Form.

  • Unless otherwise agreed in writing any third party costs reasonably incurred by Blue Croc in the course of performing the Services may be billed to the Client, unless specifically otherwise provided for in the Client Form.

Related to Client Form

  • Consent Form means the forms provided to each holder of Pre-Formation Interests to consent to the Formation Transactions and to make such holder’s irrevocable elections with respect to consideration to be received in the Formation Transactions.

  • Assignment Form means the assignment form attached as Annex 2 hereto.

  • Investment Form means a standardized form prescribed by the Management Company to be duly filled by the investor to purchase Units and will be stated in this Offering Document.

  • Enrollment Form means an agreement pursuant to which an Eligible Employee may elect to enroll in the Plan, to authorize a new level of payroll deductions, or to stop payroll deductions and withdraw from an Offering Period.

  • Opt-Out Form means the form through which Eligible Employees may note their decision to opt out of participation in the Program.

  • Exercise Form means an Exercise Form in the form annexed hereto as Exhibit A.

  • Principal of a subcontractor means (i) any individual who is a member of the board of directors of, or has an ownership interest of five per cent or more in, a subcontractor, which is a business entity, except for an individual who is a member of the board of directors of a nonprofit organization, (ii) an individual who is employed by a subcontractor, which is a business entity, as president, treasurer or executive vice president, (iii) an individual who is the chief executive officer of a subcontractor, which is not a business entity, or if a subcontractor has no such officer, then the officer who duly possesses comparable powers and duties, (iv) an officer or an employee of any subcontractor who has managerial or discretionary responsibilities with respect to a subcontract with a state contractor, (v) the spouse or a dependent child who is eighteen years of age or older of an individual described in this subparagraph, or (vi) a political committee established or controlled by an individual described in this subparagraph or the business entity or nonprofit organization that is the subcontractor.

  • Certified check means a check accepted by the bank on which it is drawn. Acceptance may be made as stated in subsection (1) or by a writing on the check which indicates that the check is certified. The drawee of a check has no obligation to certify the check, and refusal to certify is not dishonor of the check.

  • Letter of Transmittal and Election Form means the letter of transmittal and election form for use by the Corporation Shareholders with respect to the Arrangement;

  • Transfer Form means a standardized form prescribed by the Management Company to be duly filed by the investor to transfer Units and will be stated in this Offering Document.

  • FACE Form means the Funding Authorization and Certificate of Expenditure Form attached to this Agreement. The FACE Form is used for (i) requests for cash advances, direct payments or reimbursements and (ii) financial reporting by the Partner.

  • Exhibit 1 means Exhibit 1 to this Schedule C.

  • Subscription Form means the form of subscription annexed hereto as Schedule “A”;

  • Police Check means a formal inquiry made to the relevant police authority in each State or Territory and designed to obtain details of an individual’s criminal conviction or a finding of guilt in all places (within and outside Australia) that the Grantee knows the person has resided in;

  • ATTACHMENT C STANDARD STATE PROVISIONS FOR CONTRACTS AND GRANTS

  • Attachment A is an itemized list of the estimated election expenses for this Joint Election and the amounts that each Participating Political Subdivision must deposit with the Dallas County Elections Department. It also includes the Deposit Detail for each entity. The Elections Administrator will amend “Attachment A” to reflect the changing estimates of election expenses that are caused by changing circumstances and by the withdrawal of Participating Political Subdivision(s), if any, from this Election Services Contract.

  • Notice of Exercise means a notice in writing addressed to the Company at its address first recited, which notice shall specify therein the number of Optioned Shares in respect of which the Option is being exercised;

  • Exercise Agreement has the meaning set forth in Section 3(a)(i).
